To begin the new patient process please fill out our new patient forms by clicking here.

Once you have completed the new patient forms you need to call and schedule a appointment at 503-557-8444. Our office hours are 9-6 Mon-tues and thursday, and 9-2 Friday. We will also be in the office from 10-2 on Wednesday for scheduling and nutracutical purchases.

After you have scheduled a appointment make sure to show up to your appointment ontime and with your previously filled out forms.
